The inpatient diabetes educator provides timely and targeted education and support services to patients and support system. Bridges the gap between clinical care and self-mgmt. Delivering survival skills education ensures a smooth transition from inpatient to outpatient. A subject matter expert in diabetes and technology offers guidance to medical staff and participates in regular and ad-hoc education initiatives. Works collaboratively with interprofessional teams. Facilitates monthly diabetes support group. Credentialing as Diabetes Care and Education Specialist required within 1 year of hire. Must maintain 15 diabetes related CEU credits per year and active registration and licensure.

Education
Bachelors Degree Science in Nursing Nutrition or a related healthcare or life sciences field Required

Experience
2 years experience as a Registered Nurse or Dietitian Required
General Experience working with a diverse patient population Required
3 years experience in Diabetes self-management education and support experience Preferred
1 year experience in an acute care setting Preferred

Licenses
PA Dietitian -Nutritionist Required or
PA Registered Nurse License Required
Cert Diabetes Care and Ed Spec Required
