We are seeking a highly skilled and compassionate Advanced Clinical Practitioner (ACP) to join our dynamic healthcare team. The successful candidate will be a clinical expert in their area of practice providing high-quality care to diverse patient populations. Responsibilities

  • Deliver advanced clinical care across specified services including assessment diagnosis treatment and evaluation of patients with acute or chronic conditions.
  • Work autonomously while collaborating closely with the multidisciplinary team to manage patient caseloads effectively.
  • Lead and support initiatives aimed at improving patient care and healthcare services ensuring diversity and fairness in decision-making.
  • Develop and implement evidence-based clinical protocols and pathways to promote best practices.
  • Provide expert advice and mentorship to nursing staff allied health professionals and other team members.
  • Participate in clinical governance quality improvement and risk management activities to ensure compliance with healthcare standards.
  • Support a culture of equality fairness and respect among colleagues and the service user group.
  • Actively engage in continuing professional development to maintain advanced practice certification and contribute to training and educating junior staff.

Qualifications

  • Registered Nurse (RN) with an active nursing license.
  • Completion of an accredited Advanced Clinical Practitioner program.
  • Masters degree in a related field (e.g. Nursing Advanced Practice).
  • Certification and/or experience in an area of specialty (e.g. primary care acute care mental health).
  • Strong clinical decision-making and diagnostic skills.
  • Excellent communication and interpersonal skills with the ability to work collaboratively in a multidisciplinary team.
  • Commitment to equitable healthcare practices and inclusive patient care.

Day-to-day

  • Conduct comprehensive patient assessments including physical examinations and diagnostic investigations.
  • Develop and implement individualized care plans in collaboration with patients and families.
  • Prescribe medications and treatments monitoring outcomes and adjusting care as necessary in accordance with regulatory guidelines.
  • Support patients from varied cultural socioeconomic and diverse backgrounds ensuring an inclusive care approach.
  • Participate in team meetings case discussions and care planning sessions.
  • Provide health education and promotion to patients helping them achieve and maintain wellness.
  • Document all patient interactions clearly and accurately in line with legal and organizational standards.
  • Facilitate continuity of care by collaborating with external providers and community resources.
