Family Medicine Physician Outpatient / Traditional Practice (MD/DO)

Position Summary

The Family Medicine Physician provides comprehensive longitudinal primary care to patients of all ages in an outpatient setting. This role focuses on preventive care diagnosis and management of acute and chronic conditions continuity of care and coordination with specialists to deliver high-quality patient-centered evidence-based care.

Essential Duties and Responsibilities

• Provide comprehensive outpatient primary care services to pediatric adult and geriatric patients
• Diagnose and manage acute and chronic medical conditions in a clinic-based setting
• Deliver preventive care including health maintenance exams screenings and immunizations
• Manage chronic disease conditions such as diabetes hypertension asthma hyperlipidemia and cardiovascular disease
• Perform office-based procedures consistent with training and privileges
• Provide patient education regarding disease prevention wellness and treatment plans
• Coordinate care with specialists hospitals and ancillary services
• Review and interpret diagnostic tests and laboratory results
• Maintain accurate timely clinical documentation
• Participate in quality improvement population health and patient safety initiatives
• Supervise and collaborate with advanced practice providers and clinical staff as applicable
• Comply with all regulatory accreditation and professional standards

Qualifications

• Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) from an accredited medical school
• Completion of an accredited Family Medicine residency program
• Board-eligible or board-certified in Family Medicine
• Ability to obtain and maintain unrestricted state medical licensure
• DEA registration or eligibility

• Visual and auditory acuity sufficient for patient assessment
