LPN Case Manager will be responsible for Hubbardston Worcester and Rutland houses.
Job Description: -
- Coordinate medical information for admission and provide a comprehensive nursing assessment for the intake process when required by individual medical condition(s).
- Provide participant specific medical training for home care providers and nursing case training as needed.
- Monitor medical care to ensure that consumers receive quality care treatment and respect in the community.
- Maintain Individual Confidential Files and monitor placements to ensure compliance with all applicable regulations.

Qualifications:
- Current licensure to practice as a Licensed Practical Nurse by the Commonwealth of Massachusetts Board of Registration of Nursing
- Associates or Bachelors in relevant field is preferred.
- A minimum of two years prior experience with elderly or individuals with disabilities
- Proven ability to utilize electronic health record systems to monitor compliance (currently iCentrix and HCSIS)
