JOB SUMMARY
Coordinates countywide community health education activities for the agency. Promotes health-related community outreach organizes and builds coalitions to address population-based public health goals.
This position works with Healthy Living Team staff in multiple areas of chronic disease prevention and intervention. They will spend a significant portion of their time on arthritis diabetes and cardiovascular disease grant work including clinical outreach and quality improvement projects promoting the National Diabetes Prevention Program blood pressure monitoring programs etc. They will also play a role in cultivating positive and productive team relationships while supporting team members in various areas including but not limited to physical activity promotion mental well-being early childhood education center improvements breastfeeding support healthy food access and increasing child protective experiences.
MINIMUM QUALIFICATIONS
Bachelors Degree from an accredited college or university in Health Promotion Health Education Community Public Health Education or other closely related field plus two (2) years of related experience; OR a combination of related education and experience.
Due to the nature of this position the successful applicant must pass a required background check through fingerprinting and enrollment in the continuous RAP Back (Record of Arrest and Prosecution Back) program in
accordance with current County Human Resources policy requirements.
A valid Drivers License accepted by the State of Utah is required at the time of hire. A valid Drivers License issued by the State of Utah must be obtained within six (6) months of hire to operate a County vehicle.

This position typically Monday through Friday 8:00 AM to 5:00 PM with occasional evenings and Saturdays. Hybrid work schedule is available two in-office days per week (generally Monday and Wednesday) are required.
