Job Summary

Compensation Range: $73000 - $78000 yearly commensurate with experience

Benefits:

Overview:

In partnership with the Department of Youth & Community Development (DYCD) Housing Works launched the CRED Program a new specialized 24-week paid training program for justice-involved and justice-impacted individuals ages 25 40. The program provides case management connection to wraparound health services and mental health counseling support industry recognized credential trainings paid internships and employment placement and retention support. Reporting to the Program Manager the Social Worker will be responsible for but not limited to leading the case management services provided supervising two case managers providing training and clinical support to the CRED care team conducting psychosocial assessments with newly enrolled clients providing individual and group level counseling ensuring all clients have co-developed comprehensive service plans and overall ensuring CRED Program clients are connected with mental health support services needed.

Responsibilities:

Primary:

  • Oversee and lead the clinical services provided through the CRED Program.
  • Provide supervision guidance and support for the Case Managers.
  • Conduct initial and comprehensive assessments with clients newly enrolled in the program.
  • Provide individual and group level counseling services.
  • Ensure all clients have active comprehensive service plans throughout their engagement in programming.
  • Assist with referring clients to internal and external healthcare services as needed.
  • Assist program management with developing and monitoring quality assurance standards.
  • Participate in the development and implementation of quality improvement projects.
  • Engage in crisis intervention as needed.
  • Facilitate staff trainings to promote professional development and enhancing clinical interventions.
  • Utilize internal and external data systems to document client outcomes and engagement in program services.

Secondary:

  • Provide culturally competent services in accordance to Housing Works policies and practices.
  • Promote Housing Works core values consistently.
  • Ability to work with clients with diverse clients with varying levels of service needs.
  • Works collaboratively with other team members within the Justice Initiatives Department and cross-agency.

Other functions that may be required with this position include:

  • Attend and participate staff meetings and trainings as scheduled.
  • Responsiveness to emails texts and phone calls.
  • Adhere to remote workforce policies procedures and protocols.

Requirements:

  • Masters Degree in Social Work (MSW) or Mental Health Counseling required.
  • Active LMSW license preferred.

The following are qualifications and requirements of the position:

  • Minimum 3 years experience providing mental health counseling.
  • Minimum 1 year of experience supervising staff.
  • Experience working with justice-involved or justice-impacted individuals.
  • Strong written documentation and communication skills
  • Good problem-solving skills and ability to work autonomously
  • Microsoft Office Suite particularly Excel required
  • Experience utilizing EMR systems.

The following additional competencies are viewed as important to success in this position:

  • Preferably bi-lingual in Spanish.
  • Excellent written and oral communication skills.
  • Excellent or proficient technical skills to perform tasks related to Microsoft Office email and case management systems.
  • Ability to work independently and collaboratively with the team.
  • Dependability and reliability to arrive and depart timely for scheduled shift.
