ABOUT THE ROLE
Reporting to the Assistant Director of Care provides professional nursing care to residents and monitors nursing practice of Regulated Health Care Practitioners and Non-regulated Health Care Practitioners to deliver a high standard of nursing and personal care provides oversight of resident care treatments and medication administration except as warranted by the operational needs of the LTC Home (Home) provides guidance training mentoring and coordinating assigning scheduling and checking the work of assigned staff and ensuring a safe working environment assesses plans directs and co-ordinates all activities necessary for the LTC Home provides knowledge and leads the application and after-hours operational responsibilities for emergency response procedures infection prevention and control practices and protocols and occupational health and safety responsibilities within the standards of the College of Nurses of Ontario and Ministry of Health and Long Term Care Facility Program Standards.
WHAT YOULL BE DOING
- Coordinates the resident admission process which involves liaison with internal departments and disciplines physicians contracted service providers and external health care providers including hospitals community/mental health.
- Leads the admission transfer and discharge of residents to and from assigned resident home areas including consents assessments care plans and other requisite processes ensuring that all residents receive appropriate individualized care for their physical and emotional wellbeing.
- Completes resident assessments as required for new admissions and readmissions from hospital quarterly and on change in resident status in accordance with policy and standards of the MOHLTC and the Home.
- Provides guidance training mentoring and coordinates assigns schedules and checks the work of assigned staff.
- Ensures that operating staff work in a safe manner and utilize all required health and safety equipment and protective devices and follow all measures and procedures as required by the Occupational Health and Safety Act and Regulations and Regional Policies.
- Determines the appropriate actions and responses based on the MOHLTC decision trees and other guidance documentation to suspected or witnessed abuse or neglect in the LTC Home and taking appropriate and necessary action per legislation and policy in the event of a witnessed or suspected incidence of abuse or neglect in the LTC Home.
- Maintains residents records in compliance with the policies of the Health Centre the Ministry of Health and Long Term Care and the standards of the College of Nurses of Ontario.
- Participates in committees and assist with in-service education & research where applicable.
- Liaises with various community care programs to ensure necessary programs are available for clients upon discharge.
- Required to assume responsibility of the Health Centre in the absence of the Administrator and Director of Care as appropriate with on-call support from Long Term Care Administration.
WHAT WERE LOOKING FOR
- Currently in good standing and retain good standing with the College of Nurses of Ontario (CNO) of Entitled to Practice with no Restrictions as a Registered Nurse
- Minimum four (4) years nursing experience in a long-term care facility or other geriatrics environment.
- Ability to successfully complete Standard First Aid CPR-A certification within 3 months of commencement of the role (if certification is lacking or lapsed by more than 3 years) and to successfully complete an authorized CPR refresher annually
- Must have current (within the past 12 months) successful achievement of the required mandatory AIS Proficiency Evaluation Certificate for Nursing and Allied Health Disciplines and commitment to successfully completing this certification annually.
- As a condition of employment candidates will be required to provide a satisfactory Police Vulnerable Sector Check (PVSC) tuberculosis screening and proof of immunization in accordance with the Long-Term Care Homes Act (.79/10).
- Knowledge and demonstrated ability in corporate core competencies including customer focus communication collaboration and personal ownership.
