Location: Newark NY
Duration: 13 Weeks
Shift: Mid Shift 5x8 Hours (2:00 PM 10:00 PM)
Certifications Required: BLS
EMR: EPIC (Required)
Position Overview
We are seeking an experienced LVN/LPN to support a 20-bed locked dementia care unit within a skilled nursing/behavioral health setting. This unit provides specialized care for residents with dementia and behavioral health needs including those requiring behavior management and safety interventions.
The ideal candidate will have prior skilled nursing facility (SNF) experience and strong de-escalation and behavioral management skills.
Unit Details
Key Responsibilities
Provide direct nursing care to residents in a dementia/behavioral health setting.
Implement behavior management and de-escalation protocols.
Monitor and document vital signs intake/output and blood glucose levels.
Manage residents exhibiting assaultive or challenging behaviors using approved techniques.
Maintain isolation precautions when required.
Ensure proper specimen labeling and documentation.
Collaborate with interdisciplinary team members to ensure comprehensive resident care.
Float to other units as needed based on facility requirements.
Maintain accurate documentation within EPIC EMR.
Required Qualifications
Active LVN/LPN license (New York State).
Minimum 2 years of experience in skilled nursing or behavioral health.
Experience working in a locked unit or dementia care setting.
BLS Certification (current).
EPIC EMR experience required.
Ability to work every other weekend.
Must pass pre-employment health clearance and screening requirements.
