UnityPoint Clinic is a physician-led nonprofit health care organization offering comprehensive services in primary care walk-in care and specialty care. The clinical teams at the areas clinics are improving outcomes lowering the number of hospital readmissions and reducing avoidable costs associated with treating chronic conditions. The UnityPoint Clinic team is known for excellence and quality patient care putting patients at the center of their care to provide the best level of health and well-being.
Responsibilities
- Autonomously provides high-quality care to patients in their homes
- Visit types to include: Hospital at Home Urgent Care and Primary Care
- Monday-Friday 8am-5pm
- Average of 4-6 patients per day must be able to flex schedule as needs arise
- Travel Radius: 25 miles of Iowa Methodist Medical Center
- Shared call amongthe providers at a rotation of 1:3 or 1:4 (Triaged through MyNurse)
- Evaluates interprets and coordinates information to make decisions and recommendations regarding patient care
- Excellent communication (written and verbal) analytical and problem-solving skills
Qualifications
- Nurse Practitioner or Physician Assistant with preferred ICU ED or Critical Care experience
- Board Certified
- Hold a valid Drivers License
- Hold an unrestricted medical license in the state of Iowa
- Hold current and unrestricted state and federal authority to prescribe and administer medications as necessary or appropriate to the Advanced Practitioners field of practice
