Key Responsibilities

  • Ensure accurate and timely patient care documentation to support compliance and clinical quality.
  • Oversee completion and submission of Request for Services (RFS) forms to facilitate prompt approvals and authorizations.
  • Collaborate closely with VA Care Coordinators Clinical Managers (CMs) Directors of Clinical Services (DCSs) and field teams to support timely admissions and positive patient outcomes.
  • Receive review and process all VA consults and referrals with a clinical focus to ensure appropriateness of care.
  • Conduct welcome calls with patients to assess readiness for care confirm initial clinical needs and provide education.
  • Perform pre-admission chart reviews to validate completeness of physician orders and accuracy of clinical documentation.
  • Provide clinical oversight for Veterans receiving IV therapy wound care complex medication management or other specialized services.
  • Track and manage hospital admissions and discharges coordinating resumption of care (ROC) processes to maintain service continuity.
  • manage and validate all clinical orders ensuring accuracy for new admissions ROC and ongoing care updates.
  • Coordinate labs wound acs IV therapies and durable medical equipment (DME) requests to support timely interventions.
  • Partner with branch teams field sales and VA representatives to eliminate referral barriers and streamline workflow.
  • Communicate regularly with teams regarding high-acuity patients expiring authorizations and pending documentation.
  • Serve as a clinical resource on VA process requirements consult workflows and documentation standards.
  • Ensure adherence to CMS Conditions of Participation VA regulations HIPAA and internal compliance standards.
  • Participate in Quality Assurance and Performance Improvement (QAPI) initiatives through audits chart reviews and reporting.
  • Maintain awareness of patient rights infection control protocols and reporting procedures for adverse events or suspected neglect.
  • Support branch teams during surveys audits and VA reviews to ensure full compliance and readiness.

Qualifications

  • Licensed Practical Nurse (LPN) or Registered Nurse (RN) in good standing with the state Board of Nursing.
  • Strong clinical assessment skills and ability to manage high-acuity patients.
  • Knowledge of VA processes consult workflows and RFS requirements.
  • Effective collaboration with Clinical Managers DCSs and interdisciplinary teams.
  • Strong interpersonal and relationship-building skills with the ability to partner across sales operations and clinical teams.
  • Excellent organizational skills attention to detail and ability to multitask in a fast-paced environment.
  • Commitment to service excellence professionalism and Veteran advocacy.
