Responsibilities
As the LVN Quality Coordinator at Burnett Extended Care Center youll play a key leadership role in shaping a groundbreaking care experience within our newest 154-bed transitional care facility. This facility is open on the Community Regional campus Burnett is designed to deliver innovative patient-centered care across multiple specialized units including memory care and transitional care.
The LVN Quality Coordinator for Skilled Nursing provides in-depth clinical analysis on confidential patient safety peer quality and risk issues. Facilitates and coordinates performance improvement projects and data tracking from a facility-specific or system perspective. Located adjacent to the regions only Level I Trauma Center youll be integral to maintaining the continuity of care across the entire health system helping patients safely transition from hospital to extended care. Qualifications
Education
High School Diploma High School Equivalency (HSE) or Completion of a CHS Approved Individualized Education Plan (IEP) Certificate required
Completion of an accredited Vocational Nursing School program OR become licensed by equivalency required
Experience
5 years of experience in Skilled Nursing with good understanding of Quality Assurance Infection Control System Implementation and Training required
Licenses and Certifications
LVN - Current State of California Licensed Vocational Nurse License required
BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required
