You will work within the Neurosciences service line with primary office location on theunit supportingsystem-wide stroke program operations. This role provides oversight of stroke quality metrics regulatory readiness and performance improvement initiatives across the hospital and affiliated facilities.
ExpectationsWereLooking For:
We areseekinga motivated detail-oriented clinical leader to support and advance our system-wide Stroke Program.This role requires a strategic thinker who can function autonomously while influencing outcomes across disciplines.The Stroke Program Coordinator will:
Monitor analyze and report key stroke quality metrics (e.g. door-to-needle times thrombolytic compliance dysphagia screening core measures)
Collaboratively develops and implements disease management protocols/guidelines that reflect the most current research and practices in the stroke patient. Identifies develops and participates in hospital and community programs related to stroke. Develops or revises stroke written plans of care and guidelines based on accepted standards of care and current research in collaboration with physicians nurses and other interdisciplinary professionals. Communicates guidelines and plans of care to the multidisciplinary team caring for the stroke collaboration with the stroke Physician Liaison and Administrator for Quality and Clinical Integration establishes clinical benchmarks and quality outcome measures.
Lead multidisciplinary performance improvement initiatives tooptimizepatient outcomes
Ensure ongoing regulatory readiness and compliance with stroke certification standards
Coordinate stroke committee meetings case reviews and action plans
Provide education and performance feedback to clinical teams across departments and affiliated facilities
Collaborate with Emergency Department ICU Radiology Neurology Rehabilitation and system partners to ensure continuity of care
Registered Nurse license issued by the state in which the team member practices.
