Position Summary
This is an entry-level role that offers training and the opportunity to make a meaningful impact on patient care while supporting the expansion of PLHs telehealth platform. The Virtual Patient Care Coordinator (PCC) provides centralized remote support for Porch Light Healths growing telehealth services. This role is primarily responsible for patient outreach follow-up coordination and scheduling for Psychiatry and Behavioral Health with additional support for MAT telehealth as needed.
Responsibilities and Duties
As a Virtual Patient Care Coordinator you will be trained to support providers and patients through remote coordination and scheduling activities including:
- Patient Scheduling: Coordinate and schedule telehealth appointments primarily for Psychiatry and Behavioral Health services ensuring accuracy and alignment with provider direction.
- Patient Outreach: Conduct inbound and outbound calls related to missed appointments post-visit follow-ups and provider-requested correct visit type selection (telehealth phone or in-clinic) within scheduling systems.
- Insurance Verification: Verify insurance eligibility for Psychiatry and Behavioral Health services.
- Copay Collection: Collect copays for virtual services when appropriate.
- Documentation & Record Keeping: Accurately document outreach attempts appointment notes scheduling outcomes and maintain up-to-date patient demographic and insurance information within the EHR.
- Care Coordination: Act as a liaison between patients virtual providers and internal teams to support continuity of care.
- Customer Service & Patient-Centered Care: Provide compassionate professional support to patients and ensure a positive virtual care experience.
Knowledge Development
Employees are expected to develop a strong working knowledge of:
- Medication-Assisted Treatment (MAT)
- Psychiatry and Behavioral Health workflows
- Harm Reduction and Low Barrier/High Access treatment models
- Patient confidentiality (HIPAA)
- Crisis intervention and verbal de-escalation techniques
Qualifications and Skills
- Experience: Previous experience in a medical clinic behavioral health or addiction treatment setting is preferred but not required.
- Education: High school diploma or GED required; relevant experience may substitute in some cases.
Technical Skills: Basic computer proficiency and ability to learn EHR and scheduling systems. - Time Management: Strong organizational skills with the ability to manage multiple tasks in a fast-paced virtual environment.
- Dependability: Reliable punctual and able to work effectively as part of a remote care team.
Job Type & Schedule
- Full-Time
- This is a virtual/remote position (may require some in person training as need)
- Flexibility with scheduling including 8-hour shifts and rotating Saturday hours
- Wages $19-23
- Must reside in Colorado or New Mexico to be eligible for consideration.
