Job Summary

We are seeking a reliable and compassionate LPN or RN to join the team at Doctors Rehab 13034. This is a new opening for a second-shift nurse experienced in rehabilitation and post-acute care settings.

The ideal candidate will provide high-quality nursing care ensure accurate documentation and collaborate with interdisciplinary teams to support patient recovery and rehabilitation goals.

Responsibilities

Provide direct nursing care to rehabilitation patients
Administer medications and treatments as prescribed
Monitor and document patient progress
Collaborate with therapy and interdisciplinary teams
Respond promptly to changes in patient condition
Maintain accurate EMR documentation
Ensure compliance with facility policies and state regulations
Support patient and family education

Requirements

Active LPN or RN License (State-specific)
Rehabilitation or skilled nursing experience preferred
Strong clinical and documentation skills
Ability to work 2nd shift (3:00 PM 11:30 PM)
Professional dependable and team-oriented
