Job Summary
Suncoast Empath Palliative Care is seeking a compassionate and experienced APRN to provide highâquality palliative care to patients across Pinellas County with some service in South this PRN role you will conduct inâhome palliative consults perform comprehensive assessments and support advanced care planning for individuals living with serious illness. The APRN will collaborate closely with physicians and the interdisciplinary team to manage symptoms and guide patientâcentered care.

What youll Do:
Provide inâhome palliative care including comprehensive assessments symptom management and advanced care planning for patients with serious illness.
Conduct palliative consults obtain medical histories perform physical examinations and develop individualized care plans in collaboration with a physician.
Diagnose treat and monitor acute and chronic conditions while ensuring care aligns with patient goals and palliative best practices.
Serve patients across Pinellas County with some travel to South Tampa working 23 days per week in a PRN capacity.
Partner with interdisciplinary team members to support seamless compassionate care for patients and their families.
Maintain accurate clinical documentation and contribute to highâquality missionâdriven palliative care across the service area.

What Youll Need:
- Education: Masters degree in nursing or a related field.
- Licensure: Current Florida license as an Advanced Registered Nurse Practitioner certified in a practice are (if applicable to departmental requirements). Current DEA (Drug Enforcement Agency) license.
- Certifications: Valid CPR certification from the American Red Cross or American Heart Association.
- NPI provider and ability to bill CMS services or ability to obtain same (if applicable to departmental requirements).
- Minimum of two (2) years of nursing experience is required.
- Minimum of two (2) year experience as an APRN with experience in adult gerontology or oncology practice preferred.
**Location for this position is Pinellas County with some travel to South Tampa**
