Logan Health Medical Center is looking for a full-time RN or Social Worker (BSW or MSW) to join their Case Management team as a Discharge Planner!

For this role we are seeking someone with strong skills in working with complex care patients and/or experience in Emergency Department Case Management.

You will either work in a Complex Care Navigation role or as an ED Case Management role base on your individual skills.

Our Case Management team supports patients and their families by overseeing and formulating the transition plan of care from inpatient to outpatient settings ensuring seamless coordination of services for optimal patient outcomes. Youll work closely with an interdisciplinary team to develop and implement effective discharge plans fostering communication and collaboration across care teams.

RN Qualifications:

Current Montana RN license or a multi-state compact license with authorization to practice nursing within the state of Montana; required. BSN preferred.
Minimum of two (2) years RN experience in a similar position within a medical setting (e.g. physician office clinic or hospital) required. Minimum of one (1) year case management experience preferred.
Healthcare Provider (HCP) CPR ACLS and/or BLS certification required and/or as dictated by each practice/service line.

Social Worker Qualifications:

Masters Degree or Bachelors Degree in Social Work or related Social Service Field required.
Experience in a medical or hospital setting or discharge planning highly have prior social services experience preferably in a healthcare setting.
Current CPR per assigned area(s) may be required.

Both RN and Social worker require:

Excellent interpersonal skills with the ability to manage sensitive and confidential situations with tact professionalism and diplomacy.
Excellent organizational skills detail-oriented a self-starter possess critical thinking skills and be able to set priorities and function as part of a team as well as independently.
Excellent verbal and written communication skills including the ability to communicate effectively with various audiences.

Shift:

Day Shift - 8 Hours (United States of America)

Schedule: Full-time 40 hours per week. Depending on which role you fill your schedule may be as follows:

Complex Care Navigation: Five 8-hour shifts MondayFriday
ED Case Management: May be four 10-hour shifts with weekend shift rotation

Logan Health operates 24 hours per day seven days per week. Schedules are set to accommodate the requirements of the position and the needs of the organization and may be adjusted as needed.
