Job Summary
Psychiatric Mental Health Nurse Practitioner (PMHNP-BC) Geriatric Behavioral Health Mobile / Community-Based Care
Senior Doc part of Brigade Health & Affiliates is hiring a mission-driven Psychiatric Mental Health Nurse Practitioner (PMHNP) to provide high-quality geriatric psychiatry and behavioral health services in home assisted living memory care and skilled nursing settings.
If youre passionate about senior mental health dementia care integrated care models and autonomous practice this is an opportunity to build and manage your own panel while collaborating with primary care palliative care and neurology teams.

Key Responsibilities
Comprehensive psychiatric evaluations & medication management
Management of behavioral symptoms in dementia and cognitive decline
Collaboration with PCPs palliative care and facility partners
Caregiver education and relationship-based care
Documentation care coordination and panel management
What Were Looking For
Active PMHNP-BC license
Experience in geriatric psychiatry SNF ALF home-based care or community psychiatry
Strong diagnostic and medication management skills
Ability to practice independently while contributing to a team-based model
Leadership potential with interest in growing a successful patient panel
