Job Summary:
We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to provide high-quality nursing care to geriatric residents. This role involves delivering comprehensive patient care administering treatments and supporting interdisciplinary teams to ensure residents health safety and well-being.
Schedule:
- Monday Friday
- 40 hours per week
- 12-hour shifts (6:30 AM 6:00 PM)
Key Responsibilities:
- Administer medications and perform treatments as prescribed by physicians within scope of practice
- Provide specialized care including wound care tracheostomy care catheter care and other clinical procedures
- Monitor and report patient conditions treatment effectiveness and behavioral changes
- Conduct initial nursing assessments and assist with care planning and MDS documentation
- Maintain accurate and timely clinical documentation
- Collaborate with interdisciplinary teams to ensure comprehensive patient care
- Supervise and support CNAs and other staff in daily patient care activities
- Assist residents with Activities of Daily Living (ADLs) such as bathing dressing and mobility
- Participate in quality improvement initiatives and infection control programs
- Train and orient new staff members and participate in ongoing staff development
Patient Population:
- Geriatric residents requiring long-term and skilled nursing care
Qualifications:
- Active Licensed Practical Nurse (LPN) license (state-specific requirements apply)
- Strong knowledge of geriatric nursing practices and patient care
- Experience with medication administration and clinical procedures
- Excellent observation communication and documentation skills
- Ability to work both independently and as part of a team
