Float Nurse Practitioner (NP) / Physician Assistant (PA) Mobile Primary Care Senior Living Home-Based Care
Join a high-growth mobile healthcare team serving seniors across personal homes board and cares assisted living and skilled nursing facilities.
We are hiring a Float Advanced Practice Provider (NP/PA)to support a large regional footprint providing coverage for PTO turnover and new building launches while maintaining continuity of high-quality relationship-based care.
This is a high-visibility role ideal for experienced clinicians who are adaptable operationally strong and interested in future leadership opportunities.

What Youll Do
- Provide mobile primary care to seniors in homes boarding cares ALF SNF and hospice settings
- Step into established panels to ensure seamless patient continuity
- Support new market expansion and building launches
- Manage chronic conditions (HTN CHF COPD diabetes dementia polypharmacy)
- Deliver dementia-informed relationship-based care
- Collaborate with interdisciplinary partners (psychiatry palliative care neurology)
- Maintain strong documentation communication and quality standards
What Were Looking For
- Active unencumbered Nurse Practitioner (NP) or Physician Assistant (PA)license
- 2 years experience in geriatrics primary care home health SNF or senior living preferred
- Comfortable practicing independently in mobile/community settings
- Strong clinical judgment and panel management skills
- Adaptable organized and growth-oriented
- Valid drivers license and ability to travel within region
