Job Title:Prevention Services Coordinator

Program(s):SUPS The Substance Use Prevention Services helps students 11-18 learn about substance use and misuse through a prevention-oriented curriculum and community-wide awareness building activities that aim to reduce the prevalence of substance misuse in our communities.STOP- STOPSchool Violence - Focuses on preventing and reducing school violence through education crisis intervention and community collaboration.
Immediate Supervisor:Assistant Director of Prevention Services - Tracy Morrow
Minimum Qualifications: Minimum of Bachelors Degree in Human Services or related area; qualifies as a Mental Health Professional (MHP); Qualified Mental Health Professional (QMHP) preferred; Masters Degree in Human Services or Related field preferred; at least 1 year of experience in the provision of health or human services to children adolescents and adults. Preference will be given to candidates possessing experience in program development coordination and supervision.
Special Requirements:Must be 21 years of age; maintains a valid drivers license and vehicle liability insurance; operates agency vehicles and personal vehicles while driving for agency business in a safe and lawful manner at all times; maintain First Aid & CPR certification; must complete required registry clearances and criminal fingerprint background checks; schedules evening or weekend hours as required for community events and outreach; travel to off-site/community-based locations for direct service delivery; participates in a 24-hour crisis rotation.
Job duties include but not limited to
Intervenes to change attitudes beliefs and behaviors of clients to improve individual and family-level functioning.
Ensures regular updates as they relate to program engagement and programmatic needs are provided to the direct supervisor without prompting.
Provides direct support within assigned programs as needed to temporarily meet increased demand for services or when staff ratios must be maintained to ensure that program/project timelines are monitored and keep the direct supervisor informed about programmatic successes and barriers.
Communicates in a professional efficient and effective manner on behalf of the supervised program(s) to ensure that all stakeholders (i.e. employees participants families referral sources community members etc.) are kept apprised of program events requests and engagement opportunities as directed.
Coordinates communicates and seeks out opportunities to collaborate and develop partnerships with other community organizations agencies and groups; maintains networking and referral contacts with community stakeholders and other health or human service providers both internally and externally; participates in relevant community groups consortia and coalitions.
Primarily maintains assigned program records (i.e. paper and electronic records); participates and assists with compliance-related duties during program surveys program site monitoring and program audits.
Completes biweekly review of Prevention Hub and internal tracking systems to ensure programmatic documentation and completion of programmatic requirements; provides instruction to direct staff on compliance needs and ensures completion of assigned tasks.
Monitors and evaluates all program(s) and grant contracts to ensure adherence to applicable rules program standards and that contract deliverables or outcomes are continuously met; ensures submission of various reports as required by program contracts/regulations
Participates in daytime and after-hours on-call rotation; provides crisis intervention and support services and/or supervision of staff providing crisis intervention and support services.
Assists with and maintains oversight of planning for required program activities including but not limited to: National Prevention Week Youth Advisory Committee Communication Campaign Youth Prevention Education Drug Take Back Day MyGenerationRx Illinois Youth Survey School Safety Teams Key Informant Interviews Violence Prevention Tour and Family Resource Events.
Promotes and garners interest for special programmatic service offerings such as youth advisory committees recovery committee meetings area project board meetings and community committee meetings for assigned programs and provides updates regarding Agency and program information during outreach activities.
Provides a minimum of bi-weekly individual supervision to all direct reports; maintains a team-based environment to motivate and inspire staff to work collaboratively towards the Agencys mission/vision.
Monitors program budgets expenses and programmatic income; maintains understanding of program budgets and provides direction for direct staff in authorization of expenses in line with contractual requirements.
