40Summary:
The Ambulatory Care Clinical Pharmacist is responsible and accountable for the provision of safe and effective medication management for wide variety of patients with chronic health conditions.
- Patient care clinical and education services:
- Provide direct patient care and coordinate transition of care for patients with chronic health conditions
- Provide comprehensive medication management for patients with gastroenterology diseases. Provide medication counseling and education to patients and their caregivers. Monitor patient response to therapy and adjust medication regimens as needed. Collaborate with healthcare providers to optimize patient outcomes.
- Review patient charts to identify prevent or manage drug-related problems improper drug or dose selection sub therapeutic dosage over dosage adverse drug reactions drug interactions failure to receive drugs untreated indications medication use without an indication and treatment failures.
- Provide consultative services to other healthcare professionals regarding pharmacotherapy.
- Identify design and implement improvements in the medication-use system to advance patient safety and maximize therapeutic outcomes
- Assess assigned clinic care area to assure drug utilization and ensure drug access is aligned with patient care needs and regulatory standards
- Maintains proficiency in preceptor roles. Develop student resident and staff training experiences/competencies and create relationships for teaching and training opportunities
- TPN experience preferred
Qualifications:
- Graduate from an American Council on Pharmacy Education accredited school of pharmacy PGY1 residency accredited by the American Society for Health-System Pharmacy or 2 years commensurate pharmacist experience in a health-system setting.
- Previous pediatric experience preferred.
- Texas Pharmacist License or eligible for Texas Licensure as a registered pharmacist.
- Pharmacist and pharmacy preceptor licensure must be obtained within 90 days of employment.
- Current Basic Life Support for Healthcare Providers certification.
- Pediatric Advanced Life Support certification within 60 days of hire (critical care areas only).
