Summary
We are looking for a seasoned Occupational Therapist with specialized expertise in moving and handling to spearhead a 12-month initiative aimed at converting double-handed care packages into safe and sustainable single-handed care models. The role involves assessing residents currently receiving double-handed care implementing suitable assistive technologies and providing targeted training to care providers. This position is pivotal in enhancing service delivery reducing care hours where clinically appropriate improving resident outcomes and upskilling the workforce across various partner agencies.
Responsibilities
- Conduct detailed functional and moving & handling assessments for residents receiving double-handed care or those needing increased care post-hospital discharge.
- Identify and implement safe single-handed care opportunities through strong clinical reasoning and risk analysis.
- Prescribe configure and review assistive equipment such as hoists slide sheets stand aids and other moving & handling technologies.
- Develop and review person-centered care plans to ensure safety dignity independence and optimal outcomes.
- Deliver specialist training to core provider care coordinators and agencies potentially expanding to spot providers.
- Equip staff with the skills and confidence to deliver safe single-handed care techniques.
- Create and update training materials competency frameworks and guidance in line with best practices.
- Support cultural and operational change within care teams to establish sustainable new practices.
- Manage a caseload of approximately 250 residents within the project scope.
- Track progress monitor outcomes and ensure project objectives are met regarding safety user satisfaction efficiency and quality.
- Maintain accurate data and contribute to project reports audits and performance evaluations.
- Collaborate with internal teams external providers hospital discharge services and commissioning partners.
- Ensure compliance with Manual Handling Operations Regulations Health & Safety legislation and local authority policies.
- Promote safeguarding principles clinical governance standards and professional integrity.
- Maintain up-to-date documentation risk assessments and evidence-based clinical decision-making.
Requirements
Requirements:
- HCPC-registered Occupational Therapist.
- Extensive experience in moving and handling within community or hospital settings.
- Proven ability to execute safe single-handed care conversions.
- Strong understanding of assistive technology complex equipment prescription and postural management.
- Proficient in training coaching and supporting care staff.
- Experience in delivering service improvement or transformation projects.
- Excellent communication negotiation and multi-agency collaboration skills.
- Ability to manage a high-volume caseload and work independently.
- Strong analytical and problem-solving skills with the ability to evaluate outcomes and make data-driven recommendations.

We are looking for a qualified probation officer to work part time in our YJS to support transitions work for children from YJS to adult NPS services. Ideally the worker will have experience in working in Youth Justice Services prior (but not essential) and have recent work experience in NPS case management and their systems.
