Your Impact:

As a Home Health RN youll be the heart of your patients care:

Lead your caseload with autonomyassess plan and deliver care tailored to each patient
Provide compassionate guidance and support to patients and families
Coordinate care with doctors therapists and caregivers to keep everyone on the same page
Adapt plans as patients needs evolve ensuring the best outcomes
Maintain HIPAA-compliant documentation while being the trusted voice patients rely on

What Were Looking For:

Active RN license with 1 years of clinical experience
Comfort traveling within the territory (about 50% travel)
Reliable transportation valid drivers license and auto insurance
Home Health or Hospice experience is a plus
Associate or Bachelors in Nursing preferred
Experience with HomeCare HomeBase is a bonus
Sit stand bend lift and move intermittently and be able to lift 50-100 lbs.
