Mansfield District Hospital is seeking an organised and community-focused Program Coordinator to lead the coordination of our MDH at Home program.
MDH at Home delivers government-funded aged care services directly to older members of our community in the comfort and safety of their own homes. This vital program supports independence dignity and quality of life for elderly clients while ensuring access to essential care services.
In this key coordination role you will manage program intake oversee waitlists and coordinate funding arrangements in collaboration with external service providers. You will act as a central point of contact ensuring clients experience a seamless and responsive service from referral through to service delivery.
You are highly organised detail-oriented and confident working across multiple stakeholders. You understand the importance of community-based aged care and are passionate about supporting older people to remain living safely at home.
