Granite VNA Hospice is seeking an experienced Hospice RNto provide expert hospice admissions and acute inpatient-level symptom managementwithin a hospital setting. This role supports patients and families at critical moments including those who require Hospice Houselevel care delivered in hospital or alternative settings when inpatient hospice placement is not immediately you are a nurse who values clinical autonomy interdisciplinary collaboration and the privilege of guiding patients and families through complex end-of-life decisions this role offers meaningful high-impact work every day.
What Youll Do
- Lead hospital-based hospice admissions and care transitions
- Deliver advanced pain and symptom managementfor complex patients
- Provide Hospice Houselevel assessment and carein hospital or alternative settings as needed
- Serve as a trusted clinical partner to hospital physicians and care teams
- Educate patients families and clinicians on hospice philosophy and goals of care
- Advocate for comfort dignity and patient-centered decision-making

Requirements
What Were Looking For
- Licensed RN in New Hampshire (or eligible)
- Minimum 2 years of RN experience (Hospice palliative care ICU ED oncology or acute care preferred)
- Strong clinical assessment communication and critical-thinking skills
- Comfort supporting patients and families through emotionally complex situation
