Position Summary
Provide skilled physical therapy services to patients in their homes. Enjoy flexible scheduling clinical autonomy and strong interdisciplinary collaboration while making a meaningful impact in your community.
Key Responsibilities
Perform comprehensive PT evaluations
Develop and adjust individualized treatment plans
Deliver skilled therapy in the home setting
Complete accurate timely documentation
Collaborate with nurses aides and physicians
Educate patients and caregivers on safety mobility and exercises
Qualifications
Active Virginia PT license
Minimum 1 year PT experience (home health preferred)
Strong clinical judgment and communication skills
Ability to work independently
Valid drivers license & reliable transportation
