Housing Forward is a backbone organization that leads to the development of an effective homeless rehousing partnership with 100 public private and nonprofit institutions we make the experience of homelessness in Dallas and Collin Counties rare brief and non-recurring.

POSITION SUMMARY:
The Crisis System Coordinator supports the implementation of the All Neighbors Coalition Crisis System. This includes General Outreach Encampment Outreach Accelerated Shelter Placements and Complex Needs response. This role provides day-to-day coordination of crisis system activities serves as a lead for encampment-focused outreach teams and acts as the central coordinator for individuals with high-acuity complex needs within the homeless service system.
The Crisis System Coordinator maintains system-wide tracking tools facilitates recurring coordination calls and case conferences and supports cross-system collaboration with internal teams and external partners including behavioral health providers medical teams hospitals jails and prisons and housing partners. The role is focused on driving progress by reducing barriers and accelerating movement toward permanent housing or when appropriate permanent institutional care.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES
Accelerated Shelter Placements
Supports and coordinates accelerated shelter placement efforts by collaborating with outreach teams and shelter providers to identify eligible individuals facilitate referrals through the Coordinated Access System and troubleshoot barriers to placement. Ensure timely communication documentation and follow-through to move individuals quickly into appropriate shelter settings.
Encampments and Coordinated Outreach
Provides day-to-day coordination of General and Encampment Outreach efforts and serves as a lead support for outreach teams working in encampment settings. Coordinates daily outreach activities communicates priorities and updates and works closely with external partners to resolve issues in real time. Serves as a liaison between outreach teams the Housing Forward Landlord Engagement Team and Housing Case Managers to support encampment decommissioning and accelerate housing placements. Maintains accurate real-time data in Coordinated Outreach tracking tools including encampment By-Name Lists and provides regular progress updates to leadership. Supports planning and implementation of encampment decommissioning through preparation for coordination workgroup and leadership meetings.
Complex Needs Coordination
Coordinates the Complex Needs process within the homeless service system by maintaining a dynamic list of individuals with high acuity needs and tracking progress toward permanent housing or when appropriate permanent institutional care. Facilitates recurring multidisciplinary coordination meetings with behavioral health medical justice system outreach and housing partners to identify barriers assign action steps and drive accountability. Aligns partners escalates unresolved challenges and ensures consistent communication to support sustained progress for each individual.
The role participates in CAS Team meetings leads or co-facilitates outreach encampment and complex needs coordination meetings and attends Continuum of Care workgroups as required. All work is grounded in a commitment to best practices in crisis response.
KNOWLEDGE AND SKILLS:
Working knowledge of crisis system unique needs of high acuity clients and abilities to navigate across multiple systems
Familiarity with homeless service system partners including outreach teams shelter providers housing navigation behavioral health medical and justice system stakeholders
Strong coordination and facilitation skills including the ability to lead multidisciplinary meetings manage action items and drive follow-through
Ability to maintain accurate real-time data in tracking and project management tools including By-Name Lists and case coordination logs
Proven problem-solving skills with the ability to identify and resolve barriers with a lens of positivity and collaboration
Strong organizational skills and attention to detail while managing multiple priorities and time-sensitive work
Effective written and verbal communication skills with the ability to convey updates and priorities with an appropriate sense of urgency that translates to progress
REQUIREMENTS:
2 Years or more with experience in human services related work or another related field.
Must have reliable transportation valid state-issued drivers license and car insurance. Please note that local travel is expected to be frequent for this position.
Must be flexible to work at different sites as scheduled.
Must be able to lift objects that weigh up to 25 lbs.
WORK SCHEDULE
Housing Forward considers normal hours of operation Monday Friday from 8:00am-5:00pm. Some night and weekend meetings might be required to meet the needs of partner agencies or to represent Housing Forward at community meetings.
Employees are able to work both remotely and in the office. Current Housing Forward guidelines state that employees must work at least 3 days per week in the office but this is subject to change.
COMPENSATION:
The salary for this position is $53000 $72000.
Housing Forward has a flexible PTO policy for all full-time employees. The office is officially closed on New Years Day Memorial Day Independence Day Labor Day Thanksgiving Day after Thanksgiving Christmas Eve & Christmas Day. Housing Forward has MLK Jr. Juneteenth and Election Day as Days of Service.
