Job Summary
The Outpatient Physical Therapist is responsible for evaluating planning directing and administering physical therapy treatments to restore function improve mobility relieve pain and prevent disability following injury or illness. This role focuses on delivering high-quality patient-centered care in an outpatient clinical setting.
Shift & Schedule
Schedule: Day / Evening Shifts
Hours: 5 8-hour shifts
Typical Shift Window: 12:00 PM 8:30 PM
Weekdays Only: No weekends

Key Responsibilities
Perform comprehensive patient evaluations and develop individualized treatment plans
Administer therapeutic exercises manual therapy and modalities as appropriate
Monitor patient progress and modify treatment plans to achieve optimal outcomes
Educate patients and caregivers on treatment plans home exercise programs and injury prevention
Maintain accurate and timely clinical documentation in the electronic medical record
Collaborate with physicians rehabilitation staff and interdisciplinary team members
Ensure compliance with professional standards clinical protocols and regulatory requirements
Promote patient safety functional independence and overall quality of care
Required Qualifications
Active New York State Physical Therapist License
Graduate of an accredited Physical Therapy program
Preferred Qualifications
CPR certification
Prior outpatient physical therapy experience
Experience using EPIC or similar electronic medical record systems
