POSITION OBJECTIVES


The EHR Population Health Specialist will lead the implementation and ongoing management of NextGen Healthcares Population Health Management solution. This role will assist in supporting all aspects of the NextGen EHR system. The specialist will collaborate closely with the EHR Coordinator and EHR Data Analyst to support EHR operations. Additionally this position will support reporting and data analytics initiatives to improve clinical and operational outcomes.


MAJOR DUTIES AND RESPONSIBILITIES


management and configuration of NextGen Population Health solution including mapping of SNOMED codes LOINC and any other related codes.


with internal teams to ensure successful integration with existing EHR workflows.


as one of the resources for advanced EHR functionality troubleshooting and optimization.


guidance on best practices on EHR workflows related to quality measure compliance and monitoring patient care gaps.


closely with the EHR Coordinator and EHR Data Analyst to align responsibilities with reports end user support workflows and training.


with Clinical Leadership and the EHR Committee to communicate changes in quality measure specifications and affected workflows.


in developing dashboards reports and analytics to support population health initiatives.


with internal teams to ensure data integrity and compliance with HIPAA and organizational standards.


and deliver training for staff on workflows to meet measures for Population Health initiatives.


quality measure goals and troubleshoot gaps in clinical workflows.


up to date on any quality measure specifications such as GPRA HEDIS SDOH and any other KTHFS required measures and provide training on updated workflows.


documentation on quality measures for Population Health workflows.


of all ancillary KTHFS related systems and related workflows.


adherence to HIPAA in all EHR related activities.


KNOWLEDGE SKILLS ABILITIES


Ability to maintain strict confidentiality of medical records and adhere to the standards for health record-keeping HIPAA and Privacy Act requirements.


Knowledgeable of all KTHFS policies mission strategic goals and interaction with tenant activities.


Excellent English language communication skills both written and spoken.


Skill and ability to communicate in a professional manner both oral and written KTHFS health professionals and allied health workers in all KTHFS departments; federal state and local health agencies and general public.


Ability to communicate with team members and contractors from a broad spectrum of disciplines.


Able to work independently and in a team-oriented collaborative environment.


Ability to work independently and produce outcomes/deliverables per this job description.


Able to conform to shifting priorities demands and timelines.


Ability to react appropriately to project adjustments and alterations promptly and efficiently. Flexible during times of change.


Knowledge of state and federal regulations related to processes related to the use of EHR in all departments of KTHFS


Must be able to participate in and promote teamwork; be courteous to patients their family members and co-workers


Must be willing to adapt to a flexible work schedule with occasional evenings/weekends.


SUPERVISORY CONTROLS


The EHR Population Health Specialist works under the direct supervision of the Information Systems & Technology Director who provides administrative program direction. Work is assigned in terms of functional/organizational objectives. The supervisor will provide guidance with unusual situations that do not have clear precedents.


The employee plans and carries out the day-to-day activities independently resolves problems on the basis of past precedent; exercises judgment in interpreting guidelines and applicability; adhering to professional practices and legal requirements.


The employee is subjected to a variety of situations that would require using independent judgment and discretion. Assignments are reviewed for quality and compliance with established policies and procedures.


QUALIFICATIONS EXPERIENCE EDUCATION


Minimum Qualifications:


REQUIRED Bachelors Degree in relevant field; OR Associates Degree with (2) years equivalent work experience in relevant field. (Must submit copy of diploma or transcripts with application.)


REQUIRED 2 years demonstrated experience working with NextGen EHR and Population Health Management.


REQUIRED 2 years demonstrated experience with project management in healthcare IT.


REQUIRED knowledge of healthcare workflows quality reporting and population health strategies.


REQUIRED strong analytical skills and experience with healthcare data reporting tools.


REQUIRED excellent communication and collaboration skills.


REQUIRED to submit to a background and character investigation as per Tribal policy. Following hire must immediately report to Human Resource any citation arrest conviction for a misdemeanor or felony crime.


REQUIRED to submit to annual TB skin testing and adhere to KTHFS staff immunization policy in accordance with the Centers for Disease Control immunization recommendations for healthcare workers.


REQUIRED to accept the responsibility of a mandatory reporter in accordance with the Klamath Tribes Juvenile Ordinance Title 2 Chapter 15.64 and General Council Resolution #all Tribal staff are considered mandatory reporters.


Preferred Qualifications:


Familiarity with regulatory programs (e.g. MIPS HEDIS).

Experience with SQL data visualization tools or BI platforms.

Indian Preference:


Indian and Tribal Preference will apply as per policy. (Must submit tribal documentation with application to qualify for Indian Preference).
