The Care Transitions Unit (CTU) oversees this role and supports safe timely transitions of care across the continuum through integrated inpatient outpatient and virtual care models. This position is primarily aligned with the Hybrid Care APP service line a brick-and-mortar APP-driven service providing hands-on clinical management for patients requiring observation admission frequent reassessment and focused medical management.
APPs in this role practice at the top of their scope within a supported team environment guiding clinical decision-making related to expedited discharge continued observation or inpatient admission based on patient acuity and evolving clinical needs. Providers are trained in core transitional care workflows and may support additional CTU functions as the practice grows including emerging pilots and research-focused initiatives. This role involves active management of complex patients across care transitions and requires strong clinical judgment and adaptability to evolving care models.
Ideal candidates are flexible systems-minded clinicians with strong hospital-based experience who enjoy practicing across care settings and contributing to program growth. Inpatient experience is essential with outpatient or ambulatory experience valued as a complement to support facilitation of clinic-based follow-up rapid post-discharge access and proactive management of high-risk patients.

New Grads will not be considered at this time. Minimum 2 years of hospital-based APP experience required.
Nurse Practitioner: Masters-prepared graduate of an accredited school of nursing and Nurse Practitioner program. Current FL RN and APRN license and certification as a FNP ANP or ACNP.
Physician Assistant: Graduate of accredited Physician Assistant program with current PA certification by NCCPA and registration as a PA by the FL State Board of Medical Practice.
Additional Qualifications:
Active BLS certification from American Heart Association or American Red Cross at start date and DEA registration required.
