- Board Certified/Board Eligible - REQUIRED
- Active SC license & DEA - REQUIRED
- BLS - REQUIRED
- 100% CLEAN BACKGROUND - REQUIRED no exceptions
- A Self-Query NPDB report is required at the time of presentation (Should be pulled within 30 days) - REQUIRED
