Description

The Clinical Case Manager is a masters-level clinical position that integrates outpatient psychotherapy with case management services. This role is designed for clinicians with a strong foundation in case management who are developing advanced clinical skills and working toward independent clinical licensure.


The Clinical Case Manager serves as the identified clinical provider for assigned clients maintaining primary responsibility for therapeutic engagement treatment planning and coordination of services. This position provides individual therapy and clinical assessment while also delivering case management interventions to support client stability engagement and access to needed resources. The role functions under the supervision of a fully licensed clinician and supports professional growth along a defined clinical career pathway.


ESSENTIAL RESPONSIBILITIES

Clinical Services

  • Provide individual psychotherapy to assigned clients in accordance with agency standards and evidence-informed practices.
  • Conduct clinical assessments contribute to diagnostic formulation and develop person-centered treatment plans.
  • Utilize therapeutic interventions appropriate to client needs clinical presentation and stage of change.
  • Monitor client progress and adjust treatment approaches in collaboration with the clinical supervisor and treatment team.

Case Management Services

  • Provide case management services to support client access to medical behavioral health housing financial vocational and community-based resources.
  • Assist clients in navigating systems of care and coordinating services to support treatment goals.
  • Address barriers to treatment engagement including transportation housing instability benefits access and other psychosocial needs.
  • Support continuity of care through coordination with internal programs and external providers.

Collaboration Supervision and Team Role

  • Serve as the identified clinical provider for assigned clients with responsibility for ongoing therapeutic engagement treatment planning and service coordination.
  • Collaborate with prescribers supervisors and other interdisciplinary team members to support comprehensive coordinated care.
  • Participation in regular clinical supervision from a fully licensed clinician with supervision focused on case conceptualization clinical skill development ethical practice and progress toward licensure if applicable. Will also participate in trainings and interdisciplinary team meetings.
  • Integrate clinical and case management perspectives in collaboration with the treatment team to support client stability and recovery.

Documentation Compliance & Quality Assurance

  • Maintain accurate timely and professional clinical and case management documentation in accordance with agency policy Medicaid requirements and regulatory standards.
  • Use electronic medical record systems for documentation scheduling billing support and time management.
  • Adhere to all HIPAA and confidentiality requirements ensuring protection of clients protected health information (PHI).
  • Comply with OSHA standards and agency safety policies including infection control procedures bloodborne pathogen precautions and workplace safety protocols.


Requirements

MINIMUM QUALIFICATIONS

  • Masters degree in social work counseling psychology marriage and family therapy or a closely related field.
  • One of the following: Eligible for clinical licensure in New Hampshire or Masters-level clinician with relevant clinical and case management experience who practices under appropriate clinical supervision.
  • Ability to provide clinical services under supervision and integrate therapeutic and case management responsibilities.
  • Strong organizational communication and interpersonal skills.
  • Ability to work collaboratively within a team-based clinical environment.
  • Experience with clinical documentation and electronic medical record systems.

Preferred

  • Experience in case management care coordination or community-based mental health services including internship practicum or supervised clinical experience.
  • Prior experience providing psychotherapy or clinical services under supervision.
  • Familiarity with targeted case management (TCM) models and Medicaid billing requirements.
  • Experience working with adults with serious mental illness or complex psychosocial needs.
  • Knowledge of trauma-informed and recovery-oriented practices.


OTHER EXPECTATIONS

  • Maintain a valid drivers license acceptable driving record proof of auto insurance and reliable transportation.
  • Transport clients in a personal vehicle when required and appropriate in accordance with agency policy.
