We are seeking a driven professional Healthcare Referral Liaison to build relationships with hospital discharge planners social workers case managers rehabilitation centers and nursing facilities throughout Collier County.
This role is responsible for generating qualified referrals and scheduling in-home assessments with our Assessment Coordinator.
You will be the face of our 24/7 emergency home care response team.
Key Responsibilities:
Conduct daily outreach calls to:
Build long-term professional relationships
Educate discharge planners on:
Identify patients discharging within 2472 hours
Schedule in-home assessments immediately
Follow up on referrals and ensure smooth transitions
Maintain CRM documentation
Attend networking events when required
Requirements
Performance Expectations (Important)
Minimum 20 outreach contacts per day
510 referral conversations daily
35 assessment appointments scheduled weekly
Relationship-building mindset (not pushy sales)
Required Qualifications:
Experience in healthcare home care or medical sales preferred
Strong communication skills
Confident speaking with clinical professionals
Organized and CRM comfortable
Professional appearance for facility visits
Self-motivated and goal-driven

Required Skills:
Assist patients with activities of daily living (ADLs) including bathing dressing grooming toileting feeding and mobility support using assistive devices such as Hoyer lifts and transfer equipment Measure monitor and accurately record vital signs (blood pressure pulse temperature respiration) in electronic health record (EHR) systems such as Epic Cerner Meditech or eClinicalWorks Support licensed nursing staff with patient observation rounding and routine clinical tasks while adhering to HIPAA and facility protocols Provide compassionate care for patients with dementia Alzheimers disease behavioral health needs or post-acute rehabilitation requirements Collaborate with nurses physicians and interdisciplinary care teams to deliver coordinated patient-centered care Maintain infection control standards including aseptic techniques during wound care catheter care and isolation procedures Assist with patient transfers ambulation and positioning while ensuring safety and proper body mechanics Support rehabilitation and recovery programs by assisting patients with exercises mobility routines and daily activities Deliver respectful and compassionate care to diverse patient populations primarily adult and geriatric patients Document all patient care activities thoroughly and accurately within facility-approved medical documentation systems Experience & Qualifications Current and valid New York State CNA certification Previous experience as a CNA in hospitals long-term care facilities rehabilitation centers assisted living communities or similar healthcare settings preferred Experience in acute care medical-surgical units or high-acuity environments is a plus Familiarity with EMR/EHR systems such as Epic Cerner Meditech eClinicalWorks or similar platforms Knowledge of infection control practices patient safety standards and basic medical terminology Ability to work effectively within multidisciplinary healthcare teams Strong communication skills reliability and attention to detail BLS certification preferred
