Lymphatic Major Procedures Abdominal Major Procedures I&D Prostatic Abscess Prostate Procedures Urethra Procedures Ureter Major Procedures Epididymis and Spermatic Cord Procedures Bladder Procedures Kidney Major Procedures Penis Procedures Scrotum Procedures Testes Procedures Endoscopy Procedures and should be able to use holmium laser ESWL and able to perform fluoroscopy.  EMR: CPRS  Hospital Privileges required: Yes  Temporary Privileges available: Yes  Travel lodging and malpractice insurance covered Requirements:  Active unrestricted license in any State or U.S. territory  BC/BE by the American Board of Urology (ABU)  BLS  ACLS  DEA Certificate  NPI Please note: Physicians who have ever had a license restricted suspended revoked voluntarily revoked voluntarily surrendered pending action or denied upon application will not be considered for the purposes of this contract.
