Rate of Pay: 17.45 - 22.98 per hour
Summary:
As a Personal Advisor within the Care Leavers Team your primary mission is to empower young people transitioning from local authority care to independent living. You will act as a dedicated mentor and advocate helping young people navigate the complexities of adulthoodincluding housing education finance and health. By adopting a young person-centered approach you will ensure that every individual under your care has the tools and support necessary to realize their full potential and achieve a successful stable independence.
Responsibilities:
- Co-produce and update comprehensive Pathway Plans in accordance with the Children (Leaving Care) Act 2000.
- Assist young people in securing stable accommodation and maximizing their income through state benefits and budgeting support.
- Ensure access to primary healthcare mental health support and advice regarding substance misuse and sexual health.
- Act as a bridge between the young person and external agencies (utility companies health providers DWP).
Qualifications:
- NVQ Level 3 in a relevant field (e.g. Social Care Youth Work or Advice & Guidance)
Essentials:
- Substantial experience working with vulnerable young people or care leavers
- Experience in multi-agency working (Health Housing Education DWP)
- Knowledge of the Children (Leaving Care) Act 2000 and relevant safeguarding legislation
- Understanding of the challenges faced by care leavers (e.g. trauma homelessness)

- You should be available to work immediately or at a short notice.
- You should have right to work in U.K
- This role requires an Enhanced DBS
