Position Overview

We are seeking a licensed Occupational Therapist (OT) for a 13-week contract assignment in a Skilled Nursing Facility (SNF) setting. The Occupational Therapist will evaluate and treat patients to improve independence with activities of daily living (ADLs) functional mobility and overall quality of life.

Key Responsibilities

Perform comprehensive occupational therapy evaluations
Develop and implement individualized treatment plans
Provide therapeutic interventions focused on ADLs fine motor skills strength coordination and cognitive function
Train patients in adaptive techniques and use of assistive devices
Educate patients and caregivers on safety and home programs
Document evaluations progress notes and discharge summaries accurately and timely
Collaborate with interdisciplinary team members to ensure coordinated care
Maintain compliance with facility policies and regulatory standards

Qualifications & Requirements

Active State Occupational Therapist license (must be current)
Minimum 1 year of experience preferred
Skilled Nursing Facility experience preferred
Strong communication and documentation skills
Ability to work independently and as part of a collaborative rehab team
