TRANSITIONAL PROGRAMS COMMUNITY HEALTH WORKER I
Northwest Human Services (NWHS) is hiring an on-call Transitional Programs Community Health Worker to support the HOAP HOST and Outreach programs.
Location: HOAP & HOST; Salem Oregon
Job Status: On-call
POSITION OVERVIEW:
As a Community Health Worker youll be a trusted frontline advocate connecting individuals to essential health and social services. You will support clients with compassion cultural awareness and traumainformed approacheshelping them navigate challenges and access resources that promote safety stability and wellbeing.
This position plays a key role in linking clients with medical care behavioral health services housing support basic needs resources and more. Youll work closely with multidisciplinary teams community partners and vulnerable populations out in the field and within our program centers.
KEY RESPONSIBILITIES:
- Build trust and rapport with clients using culturally responsive trauma-informed and non-judgmental communication
- Support clients in navigating healthcare housing benefits food assistance recovery services and other community resources
- Provide outreach services to individuals experiencing homelessness including field engagement resource connection and documentation support
- Collaborate as an active member of an interdisciplinary care team including responding to warm handoffs and coordinating care
- Document screenings referrals and interventions accurately and timely in the electronic health record (NextGen)
- Assist clients in building support networks improving health literacy and advocating for their own care
QUALIFICATIONS:
- Associate degree in Public Health Behavioral Sciences Social Services or related field.
- At least 1 year of work volunteer or lived experience in a human service setting.
- Excellent communication computer and problemsolving skills.
- Ability to work independently manage priorities and maintain confidentiality (HIPAA).
- First Aid/CPR/AED certification or ability to obtain within 90 days of hire.
- Valid Oregon Drivers License with a driving record that permits coverage under the agencys corporate auto liability policy and personal automobile insurance preferred.
