GENERAL OVERVIEW:

ESSENTIAL RESPONSIBILITIES

• Oversee orders and inventory while maintaining quality food sources for patients (10%)

• Assist in completion and submission of wrap around services applications related to nutrition sources and outside of nutrition sources; stays current on food assistance information; Assists patients in filling out intake form. (20%)

• Counsel patients on appropriate food selections taking into consideration cultural needs and preferences; provides basic patient education and support functions based upon medical social and behavioral diagnoses. (40%)

• Maintain and updates all required database files making sure referrals are up to date and documentation in accordance to privacy laws. (10%)

• Identify individuals who require nutrition care makes appropriate referrals intervenes at basic nutrition care level as per procedures and documents process; food distribution tailored to individual health status. (10%)

• Support Healthy Food Center Garden and community events as needed.

• May be asked to cover other sites administrative functions and other duties as assigned that support the general operation.(10%)

QUALIFICATIONS:

Minimum

• High School/GED

• Act 34 Criminal Background Clearance Certificate

• Act 33 Child Abuse Clearance Certificate

• Act 73 FBI Fingerprinting Criminal Background Clearance Certificate.

Preferred

• Associates Degree

• 0 - 1 year in Food Service

• Minimum of 1 nutrition course

• Food Safety certification

• ADA registration as a Registered Dietetic Technician or registry eligible with completion of the ADA registration exam within 6 months of start date
