About Department
The Social Work Department at SVHM is recognised as a leader in developing and delivering best clinical practice to highly vulnerable groups and people experiencing all forms of minority stress as well as a widely respected Aboriginal Health Unit.
This role is an exciting opportunity for a clinician committed to practice in the dual role of health and corrections and to join the SVHM social work team who also work across St Vincents Hospital in Fitzroy St Georges Hospital in Kew and Caritas Christi Hospice in Kew.

About the Role
The role would be suited to a dynamic and compassionate social worker who has had experience in a correctional health setting as you will be a part of continuing to develop the correctional health service provided within the St Johns unit.
The successful candidate will be provided with a comprehensive and flexible professional development program as well as structured supervision and support for clinical decision making.
With a strong focus on innovative service delivery and continuous practice improvement you will also benefit from the knowledge and support of a senior team who is dedicated to research and quality improvement.
Classifications for this position will range from Social Worker Grade 3 SC31 SC34 $104135 - $118981 per annum pro rata for part time).
About You
You will have highly developed skills in assessment and complex discharge planning as well as the ability to work autonomously and as part of a multi-disciplinary team. You will contribute to positive outcomes for patients via the provision of high quality social work service using proficient clinical skills effective communication and collaboration.

Duties
Master a working understanding of the health and correctional interface.
Undertake comprehensive health screening assessment treatment management and discharge planning of allocated patients
Promote and foster clinical excellence and high standards of social work service delivery
Participate in multidisciplinary meetings case conferences ward rounds and other clinical management meetings for team based care planning.
Coordinate or contribute to allocated/agreed portfolios projects and research activities
Qualifications Skills and Experience
Bachelor of Social Work Degree/Qualifying Masters Degree
Diverse and comprehensive experience in a relevant field to the position (5 years experience as a minimum is required)
The ability to demonstrate requisite knowledge of the Victorian Correctional System is advantageous
Eligibility for membership of the Australian Association of Social Work (AASW)
