As the heartbeat of the office you will oversee the coordination of care ensure strict regulatory compliance and mentor a multidisciplinary team of field clinicians. This is a high-impact role where your clinical expertise ensures that every patient receives the right care at the right time.
Key Responsibilities
- Clinical Oversight: Supervise the daily clinical operations of a multidisciplinary team (RNs LPNs PTs OTs).
- Case Management: Review and approve plans of care ensuring they are medically necessary and tailored to patient needs.
- Compliance & Quality: Audit clinical documentation (OASIS) to ensure accuracy timely submission and adherence to CMS and state regulations.
- Mentorship: Provide clinical guidance and education to field staff assisting with complex cases or red flag clinical situations.
- Utilization Review: Monitor patient progress and utilization of services to ensure optimal outcomes and resource management.
- Collaboration: Act as the primary liaison between physicians families and field staff to facilitate seamless transitions of care.
Qualifications
- Licensure: Active unencumbered Registered Nurse (RN) license in the state of Pennsylvania.
- Experience: Minimum of 23 years of home health experience is required.
- Leadership: Previous experience in a supervisory or management role (PCM Clinical Supervisor or Team Lead) is strongly preferred.
- Expertise: Deep understanding of OASIS-E and Medicare Conditions of Participation (CoPs).
- Skills: Exceptional organizational skills and the ability to thrive in a fast-paced office environment.
