Position Overview
We are seeking an experienced Licensed Practical Nurse (LPN) to provide direct patient care in a Long-Term Care (LTC) setting in Newport VT. This role involves working collaboratively with Registered Nurses physicians and interdisciplinary care teams to deliver safe high-quality resident-centered care in accordance with organizational policies state regulations and safety standards.
Key Responsibilities
Administer medications and prescribed treatments per physician orders
Monitor assess and document resident conditions and responses to care
Assist residents with activities of daily living (ADLs)
Collaborate with Registered Nurses physicians and interdisciplinary team members
Identify and promptly report changes in resident condition
Follow individualized care plans and established clinical protocols
Maintain compliance with state regulations safety standards and organizational policies
Experience Requirements
Minimum 2 years of Long-Term Care (LTC) and/or Rehabilitation experience required
Certifications & Licensure
Active Licensed Practical Nurse (LPN) license (state-specific in good standing)
Current BLS/CPR certification
Required Skills
Strong clinical judgment communication and documentation skills
Ability to work full 12-hour day shifts and meet contract requirements
