Job Summary

The Physical Therapist evaluates plans and administers therapeutic procedures in physical medicine and rehabilitation as prescribed by a referring physician. This role establishes individualized physical therapy goals provides treatments and regularly reassesses patient progress documenting all findings accurately.

Schedule:PRN as needed schedule varies

Qualifications

  • Doctorate in Physical Therapy (DPT) required
  • 0-2 years of experience in a physical therapy setting required

Licenses and Certifications

  • PT - Physical Therapist license in the state of employment required
  • BCLS - Basic Life Support within 30 days of hire required

Essential Functions

  • Conducts thorough physical therapy evaluations to assess patient needs and develops individualized treatment plans.
  • Administers safe and effective therapy treatments ensuring all interventions are appropriately documented.
  • Reassesses and adjusts treatment plans within appropriate timeframes to align with patient progress and changing needs.
  • Provides clear concise and accurate documentation of evaluations treatment plans progress notes and billing information.
  • Educates patients and their families on treatment plans therapeutic exercises and post-treatment care to support long-term recovery.
  • Operates therapy equipment safely and maintains a clean organized and safe environment for patients and staff.
  • Collaborates with referring physicians peers and interdisciplinary team members to ensure continuity and quality of patient care.
  • Demonstrates population-appropriate competencies for all relevant patient demographics tailoring care to individual needs.
  • Maintains compliance with facility policies including safety infection control and risk management procedures.
  • Adapts to changes professionally and effectively following the chain of command to address and resolve issues impacting patient care.
  • Performs other duties as assigned.
