Were looking for passionate people to support older residents to live independently and stay socially connected by coordinating CHSP services responding to My Aged Care referrals and providing highquality customer service and information.

About theRole

  • Coordinate and deliver Home Services under the Commonwealth Home Support Program including arranging services and preparing care plans.
  • Receive action and respond to My Aged Care referrals ensuring timely appropriate service responses and strong communication with clients.
  • Provide community information support volunteers and contractors uphold customer service standards and promote social connection for older people and people with disability.

About you

  • Experience in a community servicesbased role and general understanding of the aged care sector.
  • Knowledge of aged services eligibility criteria social inclusion principles and local provider networks.
  • Understanding of Aged Care Standards Home Care Standards and relevant government agency processes.
  • Current DHS screening Child Aged Vulnerable is essential.
