Job Summary
Transition Home Healthcare is an elite home health agency serving Houston TX and surrounding areas since 2010. We are a group of passionate people dedicated to delivering the highest quality services to our patients families team members and the community. Our team is growing and looking for a dedicated:
Licensed Vocational Nurse or Licensed Practical Nurse
(LVN or LPN) - Home Health
Shifts Available:Full-Time Part Time & PRN
Service Areas:Northeast
Responsibilities:
- Collaborate with the home healthcare teamto develop and implement individualized care plans.
- Complete home care nursing visits.
- Prepare timely complete and accurate clinical notes.
- Communicate with healthcare providers and other care team members.
- Educate patients and their families about health conditions treatment plans and self-care techniques.
- Adhere to all agency policies procedures and regulatory requirements related to home health care.
Qualifications:
- Valid and current LVN or LPN license in the state of Texas or compact state.
- Minimum of one (1) year of experience in a home health setting.
- Knowledge of home health regulations and general healthcare compliance.
- Experience with HCHB EMR. (preferred but not required)
- Excellent communication and people skills to effectively interact with patients families and healthcare team members.
- Compassionate and empathetic demeanor with a commitment to providing patient-centered care.
