Function The Home Vehicle Modifications & Durable Medical Equipment (DME) Specialist plays a critical role in supporting older adults and individuals with disabilities enrolled in Medicaid waiver programs by coordinating medically necessary home modifications vehicle modifications home maintenance and chore services and durable medical equipment (DME). Working closely with Care Managers consumers physicians therapists service providers and internal review committees this position ensures that all services are person-centered medically appropriate cost-effective and compliant with Medicaid waiver and AOoA program standardshelping individuals remain safe independent and supported in their homes and communities. What Youll Do Service Coordination & Documentation - Coordinate Home Modifications Home Maintenance and Chore services Vehicle Modifications and DME requests for waiver program participants.
- Obtain physician orders physical therapy evaluations and medical documentation required to support service and equipment requests.
- Collaborate with physicians therapists and Care Managers to ensure evaluations and documentation are complete relevant and appropriate to the request.
- Review documentation to confirm medical necessity and program compliance.
- Determine when homeowner authorization is required and obtain signed homeowner letters; verify ownership through the County Auditors website.
Provider Coordination & Authorization - Request bids from a minimum of three (3) qualified providers when available and follow up to ensure bids are timely complete and compliant.
- Collaborate with Care Managers and consumers on bid selection and provide status updates throughout the process.
- Prepare complete case proposals for Best Practice Committee review including medical documentation drawings or diagrams material lists and cost breakdowns.
- Present cases to the Best Practice Committee and complete follow-up actions based on recommendations.
- Verify consumer status with the Care Manager before authorization to ensure the individual is not hospitalized or residing in a nursing facility.
- Award bids in accordance with committee or Care Manager recommendations and notify all parties involved.
Monitoring Tracking & Follow-Up - Track all Home Modifications Home Maintenance and Chore services Vehicle Modifications and DME from initiation through completion.
- Place and manage the $1 placeholder in the service plan (PCSP) until project completion and consumer satisfaction is confirmed.
- Obtain and document consumer satisfaction in compliance with program requirements and communicate outcomes to the Care Manager.
- Complete reconciliation of authorized services including accurate entry of Care Managerapproved service amounts.
Collaboration Compliance & Support - Conduct home or community visits as required to obtain documentation or meet case management contact schedules.
- Serve as backup support for Care Manager Assistant functions as needed.
- Maintain working knowledge of approved DME Home Modifications Home Maintenance and Chore services and Vehicle Modifications.
- Maintain confidentiality and comply with HIPAA and AOoA policies.
- Promote positive customer service teamwork and respectful communication with consumers caregivers providers and staff.
- Utilize Microsoft Outlook Word Excel eFax and other required systems to accurately track and manage cases.
- Meet physical requirements of the position including lifting up to 20 pounds and navigating stairs.
Additional Responsibilities - Provide education to consumers and providers regarding required processes and expectations.
- Utilize agency-provided equipment including dual monitors to access multiple databases simultaneously.
- Maintain reliable transportation a valid drivers license proof of insurance and the ability to pass annual Motor Vehicle Record (MVR) checks.
- Maintain organized accurate individual case files.
- Participate in training and professional development activities.
- Contribute to divisional and agency goals.
- Perform other duties as assigned.
Authority & Reporting Relationship This position reports directly to the Assistant Vice President (AVP) of PASSPORT. This position does not have supervisory responsibility. Qualifications - A bachelors degree is preferred in human services healthcare or a related medical field.
- Experience coordinating services in a community-based or healthcare setting preferred.
- Strong organizational time-management and prioritization skills.
- Knowledge of medical terminology and basic disease processes.
- Ability to work in consumers homes hospitals and nursing facilities.
Special Requirements & Conditions - Continued employment is contingent upon the availability of funding for AOoA Long-Term Care Programs.
