Job Summary

The Community Health Worker is a trusted peer with lived experience inHIV-affected communities responsible for delivering comprehensive HIVprevention services through outreach education testing and navigation. Thisposition focuses on addressing social determinants of health and prioritizingkey populations such as Black and Latinx/Latine MSM cisgender and transgenderwomen people who inject drugs and their partners and youth under 24. The CHWintegrates into multidisciplinary teams to promote health equity reducestigma and support the Ending the HIV Epidemic plan using a status-neutralsyndemic approach.

ESSENTIAL DUTIES AND RESPONSIBILITIES: Coreduties and responsibilities include the following. Other duties may beassigned. Conducts prioritized community outreach to engage high-riskindividuals in HIV prevention education and awareness. Performs rapid testingfor HIV HCV HBV and STIs and administers brief screenings for substanceuse mental health and food insecurity. Provides product-agnostic education onlong-acting prevention options like PrEP and PEP. Navigates clients tobiomedical prevention supportive services and primary care includingfollow-up and barrier removal. Develop and distribute culturally responsiveeducational materials for in-person events and stigma reduction advocacy training for community members to empower peer educationon HIV prevention. Collects and enters de-identified data into systems like Excelor Athenahealth for program evaluation and reporting. Participates in trainingon motivational interviewing SBIRT and cultural humility. Collaborates withpartners such as Florida Department of Health and local EHE coalitions. Upholdsconfidentiality HIPAA compliance and agency values of equity and stewardship.

SUPERVISORY RESPONSIBILITIES: This job has nosupervisory responsibilities but may mentor volunteers or peers in outreachactivities.

QUALIFICATIONS: To perform this job successfully an individual must be ableto perform each essential duty satisfactorily. The requirements listed beloware representative of the knowledge skill and/or ability required. EDUCATION AND/OR EXPERIENCE: High schooldiploma or equivalent required; Associates degree in public health or relatedfield preferred. 1 years experience in community outreach peer support orHIV prevention. Lived experience with HIV PrEP or related challenges inprioritized communities strongly preferred. Certification as a CommunityHealth Worker or willingness to obtain within 12 months. Familiarity withrapid testing screening tools and navigation services. Knowledge of HIVsocial determinants of health and local resources. Experience with dataentry software like Excel or Athena Health. Willingness to travel foroutreach events.

LANGUAGE SKILLS: Ability to read and comprehendprofessional health materials author correspondence and memos. Ability towrite effective correspondence. Ability to effectively present information inone-on-one and small group situations to customers clients and otheremployees of the organization. Bilingual English/Spanish and/orHaitian-Creole/English preferred.

MATHEMATICAL SKILLS: Ability to add andsubtract two-digit numbers and to multiply and divide with 10s and to perform these operations using units of American distance volumemoney and weight measurement.

REASONING ABILITY: Ability to apply commonsense understanding to carry out detailed written or oral instructions. Abilityto deal with problems involving a few concrete variables in complex situations.

COMPUTER SKILLS: To perform this jobsuccessfully an individual should have knowledge of health data softwareInternet software; Spreadsheet software and Word Processing software.
