The Families FIRST program at Terros Health assists families involved with the Department of Child Safety. Working as part of a team the Care Coordinator will impact the lives of families trying to overcome the barrier of substance abuse to reach family reunification and self-sufficiency. This individual will work closely with DCS the clinical team and other providers to coordinate and help families be successful in their DCS case plans. Part of their responsibility is to provide monthly meetings to assure that goals are being set and achieved. They will also empower families by providing resources and skills to show stability in their homes. The individual will need to be professional friendly a self-starter organized and compassionate.

Under Terros Healths policies and professional requirements with oversight from clinical staff the Outreach Specialist/Care Coordinator works closely with DCS collaborative partners and other community resources to engage patients/residents in appropriate services conduct outreach as needed make referrals and enhance independent living skills. This position reports to theAFF Care Coordinator Manager.

Essential Duties Include:

  • Responsible for initial outreach for all minors and TAY patients referred to AFF. This includes having patients and/or guardians sign initial paperwork and schedule or coordinate scheduling of AFF assessment.
  • Responsible for ongoing care coordination for TAY clients referred to AFF to include but not limited to:
    • Complete a Short-Term Service Plan within three days of assessment and upload it into Guardian.
    • Conduct a meeting or conference call with the Service Team (patient family members DCS worker and other agencies providing services to the family) to discuss and finalize the Service Plan within 15 days of the comprehensive assessment.
    • Schedule and conduct a monthly Service Plan review with the Service Team to monitor progress identify completed goals or significant changes update the Service Plan quarterly and upload document into Guardian.
    • Upload required documents into Guardian within prescribed scope of work time frames.
    • Monitor the random alcohol and drug testing plan for assigned patients per the AFF drug testing procedures. Coordinate with the Extended Foster Care Coach and/or DCS specialist on missed tests positive tests and communicate needed changes to the drug testing plan.
    • Provide concrete supportive services based on the needs of the young adult in achieving sobriety including but not limited to car repair housing assistance utility assistance clothing food phone etc.
    • Conduct outreach and re-engagement activities as described in the AFF Program Manual prior to closing a case.
    • Complete required documentation in patient records that conforms to professional practice state licensing regulations and company procedures related to Progress Notes with an expectation to meet production metrics.
    • Ability to drive to various locations throughout assigned territory to visit and provide services to patients.

Qualifications
  • Education:
    • An associates degree in human services or behavioral health Field and at least three years of related experience including working with multi-problem families in the field of substance abuse child welfare or behavioral health; or
    • A bachelors degree or higher in a behavioral health field.
  • Knowledge in use of electronic health record. NexGen a plus.
  • Knowledge of word processing systems i.e. Microsoft Word Access and Excel. Ability to type 55 wpm.
  • Ability to read and compose documents in a professional and technically correct manner.
  • Must possess good communication skills and be able to work well with individuals and groups.
  • Must demonstrate time management organization conflict resolution and judgment skills.
  • Working knowledge of the child welfare/child safety system.
  • Must have valid Arizona drivers license be 21 years of age with minimum 4 years driving experience and meet requirements of Terros Healths driving policy. This role is a driving position.
  • Must be able to pass the Arizona Department of Child Safety Direct Service Central Registry Clearance without any disqualifying events.
  • Must have a valid Arizona Fingerprint Clearance card or apply for an Arizona fingerprint clearance card (Level 1) within 7 working days of assuming role.
  • Must pass a TB Test.
