Job Summary

Job Summary:The Housing Specialist as a vital member of the Assertive Community Treatment (ACT) team is responsible for coordinating and implementing housing services for individuals with severe mental health challenges. The specialist collaborates with team members housing providers and community partners to ensure clients have access to safe affordable and supportive housing options.

Responsibilities:

  1. Assessment and Planning:

    • Conduct comprehensive housing assessments to identify clients housing needs preferences and barriers.
    • Develop individualized housing plans in collaboration with clients considering their unique circumstances and treatment goals.
  2. Housing Placement:

    • Establish and maintain relationships with local housing providers landlords and housing authorities.
    • Facilitate the process of finding and securing suitable housing options for clients including supportive housing and independent living.
  3. Tenant Support:

    • Provide ongoing support to clients in navigating the responsibilities of tenancy including rent payments lease agreements and communication with landlords.
    • Assist clients in accessing and maintaining government assistance programs such as housing vouchers.
  4. Crisis Intervention:

    • Collaborate with the ACT team to address housing crises promptly and effectively.
    • Develop crisis intervention plans related to housing instability and homelessness.
  5. Advocacy:

    • Advocate for clients housing needs within the ACT team and with external agencies.
    • Work with housing providers to ensure that accommodations and support services are tailored to clients mental health and recovery needs.
  6. Collaboration:

    • Work closely with other ACT team members including clinicians case managers and vocational specialists to integrate housing goals into overall treatment plans.
    • Participate in team meetings to share updates on clients housing status and address any barriers.
  7. Documentation:

    • Maintain accurate and timely documentation of housing assessments plans and client interactions.
    • Ensure compliance with confidentiality and documentation standards.

Qualifications:

  • Bachelors or Masters degree in social work psychology counseling or a related field.
  • Relevant experience working with individuals with severe mental health challenges and housing instability.
  • Knowledge of local housing resources housing laws and programs supporting individuals with mental health needs.
  • Strong interpersonal and communication skills.
  • Ability to work collaboratively within a multidisciplinary team.
  • Understanding of trauma-informed care and recovery-oriented principles.
