About PACE:

Philanthropists Gary and Mary West founded West Health with the mission of enabling seniors to successfully age in place with access to high-quality affordable health and support services that preserve and protect their dignity quality of life and independence. The Gary and Mary West PACE (Program of All-inclusive Care for the Elderly) affiliated with West Health () is a non-profit that delivers medical social and behavioral services to the frail elderly to the North County of San Diego. This PACE center is entrepreneurial in nature and focuses on innovations and bringing new processes to the traditional PACE environment.

Position Summary:

The Supportive Housing (SH) Coordinator serves as a trusted on-site presence at La Sabila a newly developed supportive housing community for older adults. This role supports residents in successfully transitioning into housing stabilizing after moving in and remaining safely housed by connecting them to supportive services community resources and health-related supports such as PACE.

The SH Coordinator works collaboratively with housing partners service providers and West PACE Marketing Community Outreach and other interdisciplinary team members to reduce isolation improve access to resources and promote successful aging in place. This position is non-clinical and non-property management focused emphasizing relationship-building dignity independence and housing stability.

Essential Functions:

Resident Engagement and Housing Stabilization

  • Serve as a welcoming consistent point of contact for residents at La Sabila.
  • Support residents during move-in and early tenancy through orientation check-ins and warm handoffs to service providers.
  • Conduct regular community outreach to identify emerging needs related to housing stability safety and well-being.
  • Support residents in navigating life in permanent supportive housing while reinforcing autonomy and choice.

Resource Coordination and Referrals

  • Connect residents to community-based services including benefits assistance food access transportation behavioral health aging services and social support organizations.
  • Collaborate with property management and housing partners to support tenancy success while maintaining clear role boundaries.
  • Identify eligible residents who may benefit from additional care coordination including referral to PACE when appropriate.
  • Maintain strong working relationships with local service providers housing partners and public agencies.

Community Building and Engagement

  • Plan and facilitate on-site engagement opportunities such as coffee chats welcome gatherings resource workshops and social connection activities.
  • Foster a sense of belonging and community among residents.
  • Apply trauma-informed culturally responsive engagement practices.

Documentation Data & Reporting

  • Complete required documentation accurately and in a timely manner as outlined by GMWP policies and procedures.
  • Enter resident-level data outreach activities and service connections into HMIS (Homeless Management Information System) as required.
  • Support reporting requirements for housing and supportive services programs in collaboration with internal team members and external partners.

Compliance and Professional Standards

  • Complete RTFHSD Clarity Training within required timeframes and maintain compliance with ongoing training requirements.
  • Demonstrate proficiency or willingness to be trained in HMIS data entry and reporting standards.
  • Uphold confidentiality data privacy and information-sharing protocols.
  • Operate within the defined scope of a supportive services role (non-clinical non-property management).
  • Represent Gary and Mary West PACE with professionalism compassion and integrity.

Education and Certification:

  • Associates degree or equivalent experience in social services human services community health housing or a related field
  • Bachelors degree in social work gerontology public health or related field preferred

Skills and Experience:

  • Minimum of 2 years of experience working with older adults supportive housing vulnerable populations or community-based services.
  • Experience with or willingness to learn HMIS data entry and reporting.
  • Ability to complete required compliance training courses including RTFHSD Clarity Training.
  • Strong interpersonal skills and ability to build trust with diverse populations.
  • Excellent communication organization and follow-through skills.
  • Ability to work independently in a community-based setting.
  • Familiarity with Medi-Cal Medicare PACE and other aging services systems preferred.
  • Experience working within trauma-informed and housing-first models preferred.
  • Bilingual (English/Spanish) strongly preferred.

Medical Clearance:

Employees must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
