Role specifics and benefits:
As a Home Health Registered Nurse youll deliver comprehensive patient care in home settings collaborating with families and the broader care team to create and implement individualized care plans. Your responsibilities include thorough initial assessments and ongoing monitoring of vital signs and symptoms; wound care and infection prevention; IV therapies and medication management; chronic disease management and education for patients and caregivers; fall prevention and safety optimization in the home; palliative and comfort-focused care when appropriate; timely documentation and communication with physicians case managers and social workers; and escalation of changes in patient status to ensure prompt quality interventions. Youll coordinate services with physical therapy occupational therapy and other disciplines to support functional goals while empowering patients to participate actively in their care. Documentation is precise compliant and timely to support accurate continuity of care and reimbursement. Your role will also involve travel between patient homes which is supported by our 24/7 clinical and operational teams so you never navigate complex days alone.
This position offers competitive compensation designed to recognize your expertise and commitment. The weekly pay range is $2039 to $2116 with guaranteed 40 hours per week supporting financial stability as you plan your addition to competitive pay youll have access to a bonus program housing assistance and extension opportunities that allow you to continue providing exceptional home-based care while advancing within the company. The contract starts on 01/21/2026 and runs for several weeks with possibilities for extension based on performance patient needs and organizational requirements.
